Registration-Step 6 of 6
For your application to be complete, the USYS Membership Application must be signed by a parent or legal guardian in person, and your child's birth date must be verified by the SAYSO, league, and district Registrars. To complete the registration process, you must either (1) visit the SAYSO office to sign the application and to provide additional items, or if there is time, (2) mail in the completed application and additional items. If you want to complete the registration by mail, you must begin the process approximately 7-10 days prior to the first practice. Players whose incomplete applications are in transit may not participate in SAYSO activities until all registration steps are completed. Please note that SAYSO cannot mail your uniform following receipt of the completed application.
Step 6(a): Gather the following two items.
* proof of age:
Locate you child's proof of age: birth certificate, passport, driver's license,
U.S. alien registration card, INS certificate, or American Citizen Born Abroad
certificate. (Hospital,
baptism, and religious certificates are not accepted by CYSA.) You don't need
to bring the
"original" - a photocopy is acceptable. If you do bring an original, SAYSO will make a photocopy of whatever
you provide and return the original to you. If the document is not written in English, a translation by a native speaker is
required.
* picture of your child's face for the player pass:
SAYSO prefers that you email a
digital facial image - any size and any resolution. However, if you want
to provide an actual photograph, SAYSO will accept that. (If you don't have
a digital image or a photograph, SAYSO can take one at the SAYSO office. Locate a recent, wallet-sized
COLOR photograph showing your child's face. A school or the previous year's team photo
is
ideal. A candid shot from your family collection is acceptable if the face is
the predominant feature in the photo and the photo is recent. The face must
fit in a 1-inch wide by 1-inch high box. That is the size that the
photo will be cut to. Pictures must show the face from hair to shoulders. The pose is
unimportant. However, no hats or sunglasses, are permitted. Instant (Polaroid) photos from
your own camera or from the arcade are not acceptable.
Pictures must be from a “real” camera. Cut
the photo so that it is exactly 1” wide and 1” tall, leaving the face (from hair
to shoulders) showing.
Write your child's first and last name on the back of the photo.
Step 6(b): Check SAYSO's office hours and directions. (Use your browser's "Back" button to return here.)
Step 6(c): Visit the SAYSO office to sign and/or complete the USYS Membership form, show the proof of age, provide the picture, and submit payment (if you haven't already paid online). Don't forget! - You are on the "waiting list" until you your USYS Membership Application is signed, complete, and paid for. And, when SAYSO is full, SAYSO is full.
Step 6(d) (optional): Take the registration voucher and discount coupon (that will be given you when you visit the SAYSO office) "next door" to Kicker's Soccer Shop for any of the soccer gear that you might need.
