Instructions for
Filling out the
USYS Membership (Registration) Form 1601
(Revised 6/18/05)
All SAYSO players register with United States Youth Soccer (USYS) through the Redwood Junior Soccer League, Inc., which is affiliated with the California Youth Soccer Association (CYSA). The following instructions are distilled from instructions issued by the Redwood Junior Soccer League, Inc. and modified for use by players on SAYSO teams.
1A. Obtain a recent, wallet-sized COLOR photograph of your child's face. School or team photos are ideal. Candid shots from your family collection are acceptable if the face is the predominant feature in the photos and the photos are recent. (The face must fit in a 1-inch wide by 1-1/2-inch high box. That is the size that you will cut the photo to.) The pose is unimportant. However, no hats and no sunglasses. Pictures must show the face from hair to shoulders. Instant (Polaroid) photos from your own camera or from the arcade are not acceptable. Pictures may be printouts from your digital camera or high-quality, color scans of photos you don't want to part with. You may also submit a digital file via email to SAYSO.
1B. Cut
the photo so that it is exactly 1” wide and 1-1/2” tall, leaving the face
(from hair to shoulders) showing.
1C.
Write your child's first and last name on the back of each photo.
2. NEW PLAYERS ONLY (Returning players skip to step 3.)
2A. Obtain a copy of your child's birth certificate, passport, green card, DMV ID card, or driver's license. To register initially, this does not have to be an official, county-issued embossed copy. A photocopy is fine. However, eventually you will also need an official copy. Do not provide your only ORIGINAL, as it will not be returned. Hospital baptismal certificates are not accepted by the league.
2B. If your birth certificate is not written in English, a foreign translation is required. Contact the SAYSO office for more information.
3. Complete
the USYS Membership Form 1601. Use a BLUE ballpoint pen. Do not use pencil.
Press hard. (Make sure that you are using the 1/04 version of Form 1601. Older
versions of this form may not be accepted.

Complete
all of the yellow part of the form except for the gray “OFFICIAL USE ONLY”
box.
Unless
your child played club soccer last year, the Number of Prior Seasons played
is zero.
Leave the remainder of that line blank if he/she did not play club soccer.
Signatures:
TWO parent signatures are required: “IMPORTANT” statements and the
"CONSENT FOR MEDICAL TREATMENT (MINOR)". If you are using photocopies, pdf
printouts, or other computer-generated forms, you must sign BOTH pages (the
1601 is a 2-part form) in BLUE ink. You cannot sign one and photocopy the
other.

4. Write a
check to "SAYSO" or "SAN ANDREAS YOUTH SOCCER ORGANIZATION" for the
registration fee. Note that this amount covers your league and USYS membership
fees.
5. Paperclip
(NO STAPLES!) everything together.
Return all of the photos, birth certificate
or passports (new players only), check, and the application (all three parts)
to your team manager. If you must, you may mail everything to the SAYSO
Office, 1250 Fifth Avenue, Suite 123, Belmont, CA 94002.
6. Please
start the process to get an official copy of your birth certificate (if you
haven’t already done this in some previous year).
