Completing and submitting this application are not sufficient for acceptance into SAYSO programs. Your signatures are required. Most of the information is required. Important: The name of registration (which appears on the player pass) must be EXACTLY the same as the name appearing on the document used as proof of age (e.g., copy of birth certificate, passport, etc.) So, list first names as "Victoria," not "Vickie; "Samuel," not "Sammy," etc.
