Is my child covered by medical insurance?

All SAYSO players become "members" of (1) United States Youth Soccer (USYS) through the California Youth Soccer Association (CYSA), (2) US Club Soccer (USCS), or (3) both. USYS and USCS members are covered by accident insurance. The insurance generally covers players, coaches, managers, and other categories during sanctioned practices, games, and tournaments, and travel to and from these events.

"US Club Soccer provides the following insurance coverage for its registered member clubs, teams, academies, and leagues. Individuals covered: US Club Soccer staff, registered players, registered coaches and administrators, referees and volunteers of its members while participating in covered soccer activities." The US Club Soccer accident insurance plan includes the "Covered Persons" during sanctioned and supervised activities of US Club Soccer, such as tournaments, games, and practices. In addition, group travel to and from a sanctioned activity is covered, provided the covered individuals are traveling with the soccer team or club, and are under the direct and immediate supervision of the team or club. Coverage applies outside the United States if travel has been approved by US Club Soccer. Coverage applies within the United States without the issuance of travel approvals or permissions.

Are players trying out for CYSA covered by medical insurance?

"Children trying out for CYSA soccer would be covered under our [i.e., CYSA] insurance, whether they are currently registered or not, for the reason that our [i.e., CYSA] insurance covers the "operation" of CYSA and its leagues, not just each individual registrant. This holds true because of our [i.e., CYSA's] "continuous" coverage from one year to the next. The only exception to this would be if an entire league has not been a member of CYSA and is just joining as of Sept 1. Coverage would not apply any earlier than that for them." What do I have to fill out to be covered?

There is nothing to fill out except the USYS and/or USCS membership (registration) forms that are submitted at the beginning of the season.

What are the details of the insurance plan?

The USYS insurance plan is a "full excess" plan. That means the insurance covers all expenses that your regular medical plan do not cover, including your deductibles. If you have no medical coverage at all, then the USYS insurance will cover all of your medical expense. There is a $100 deductible for the plan, and the maximum medical benefit is $300,000.

The USCS plan is a "full excess" plan. The benefits are payable in excess of any other health care plan, (as defined in the policy) regardless of any "coordination of benefits" provision contained in such health care plan. The medical expenses must be incurred within 52 weeks (1 year) of the date of injury and be reported within 90 days of the date of injury. A deductible of $250 will apply for each covered injury. Claims will be paid on a usual and customary basis. The General Liability and Accident Plans are underwritten by Markel Insurance Company, “A” rated by A.M. Best’s Rating Service.

Where can I get a certificate of insurance?

Schools, school districts, and city parks and recreation departments (but, not parents!) may obtain named certificates of insurance covering SAYSO's use of their fields. Contact the SAYSO office.

How do I make a claim?

Claims for SAYSO players covered by USYS (i.e., CYSA) insurance are initiated by completing a Youth Soccer Case Report. This form is also in the printed "Team Manual" distributed to every coach. The Youth Soccer Case Report should be submitted after any significant injury, regardless of expected or eventual costs.
