A photocopy of your Positive Coaching Alliance wallet or membership card.

A small picture of your face. Please trim to 1" wide x 1-1/2 high and write your name on the back. You may also submit a digital file via email to SAYSO.

You will also need to complete the following four forms. Most of these forms can be filled out onscreen and then printed out. The Word versions can be saved on your own computer for you to use next year (as long as the forms don't change!). The pdf versions can be filled out onscreen, but they cannot be saved unless you have Adobe Acrobat or similar. (If you need it, you can obtain the free pdf viewer here. This viewer will permit you print but will not permit you to save changed pdf documents.)

Be sure to prepare the CYSA-N coach registration form in duplicate (requires 2 signatures).
