Adult Pass Card
Application
Looking for a form to register as a coach,
assistant coach, trainer, or team manager and get your adult pass card? The
process starts with an online (not a printed) application.
Click here to register
as an adult in CYSA. After you've entered your personal information, use the
"Forms" tab within the registration system to print out the applications.
Then, sign and bring them to the SAYSO office.
Click here to register as an adult in US Club Soccer. Within the
green "Registration" tab, choose the "Staff Background Check" option. After
you have registered online, tell SAYSO so that we can request your pass
card. |
