Tournament and jamboree applications cannot be faxed or emailed to SAYSO.
For CYSA-N tournaments

(  ) Don't fold anything. Don't seal anything. Don't put anything inside any of the envelopes. Don't staple anything. Paperclip everything together.
(  ) Four copies of the CYSA Tournament Application
(Distribution: 1 to tournament after district approval, 1 to D2 tournament coordinator, 1 immediate courtesy copy to the tournament, and 1 for SAYSO)
(  ) Two #10 envelopes, with 1-oz postage, addressed to the jamboree
(Distribution: 1 immediate courtesy copy to the tournament, and 1 for mailing the application after district approval)
(  ) OPTIONAL: If you want the tournament to notify you when it has received the application forwarded by the district, you can include a self-addressed, stamped envelope or postcard. Even though you've included it, you might not receive the application acknowledgement back since the tournament is not required by CYSA to perform this courtesy.
(  ) A note to SAYSO indicating the amount of the check needed and the name of the Payee
(  ) One #10 or medium-sized envelope, with 2-oz postage, addressed to the District 2 Tournament Coordinator
(Use: To mail everything to the district for approval.)

For CYSA-N jamborees

(  ) Four copies of the jamboree application. This form is unique to each jamboree. If the jamboree does not have one, use the CYSA Tournament Application.
(Distribution: 1 to jamboree after league and district approvals, 1 to D2 tournament coordinator, 1 courtesy copy to the jamboree, and 1 to SAYSO)
(  ) Two copies of the District 2 Jamboree Application Form
(  ) Two #10 envelopes, with 1-oz postage, addressed to the jamboree
(  ) OPTIONAL: If you want the tournament to notify you when it has received the application forwarded by the district, you can include a self-addressed, stamped envelope or postcard. Even though you've included it, you might not receive it back.
(  ) One #10 or medium-sized envelope, with 2-oz postage, addressed to the District 2 Tournament Coordinator
(  ) One large envelope to hold everything and addressed to: "RWL Registrar" (No address or postage is needed.)
(  ) A note to SAYSO indicating the amount of the check needed and the name of the Payee

For CYSA District II Cup

(  ) Two copies of the online Tournament Registration Form printed out from the website where you registered your team. There is no other application form.
(Distribution: 1 to tournament, and 1 for SAYSO)
